Safari on iOS or MacOS:

Go to iOS Settings
Find Safari, tap into it.
Under "Privacy and Security" section, set "Prevent Cross-Site Tracking" to Off.
Go back to the form and refresh the page.

Chrome on iOS:

Go to iOS Settings
Find Chrome, tap into it.
Set "Allow Cross-Website Tracking" to be On.
Go back to the form and refresh the page.
